About the Role

You'll provide clinical and operations support to seniors living in and around Chubbuck and Pocatello, ID, working closely with an assigned Savoy Life RN and provider. You'll travel between various senior living settings such as assisted living, independent living, and memory care units, as well as individual homes, to help coordinate and manage care. On the back-office side, you'll schedule patient visits in the EHR, answer inbound calls and messages, manage faxes, enter and organize patient records, and help identify health plan insurance for residents. On the care coordination side, you'll support the RN and Advanced Practice Provider in driving patient outcomes, collaborate with community resources, manage facilitated telehealth visits, lead resident visits, and build positive relationships with patients, caregivers, families, and healthcare partners. You'll also help engage residents and families through education about Savoy's offering to get consent forms signed.

Requirements

  • High School Diploma / GED (or higher)
  • 2+ years of experience in healthcare with a focus on care coordination and healthcare administrative support
  • Medical Assistant, Patient Care Technician, or equivalent certification preferred, otherwise 5+ years of healthcare experience
  • Experience in geriatric care or senior living community is highly desirable
  • Must possess a valid and current driver's license and be comfortable driving to different senior home settings
  • Excellent electronic medical record (EMR) documentation skills and comfort with various technologies
  • Ability to juggle many tasks and switch between resident visits and behind-the-scenes coordination
  • Flexibility to adapt to a dynamic and growing startup organization as the role and company evolve

Responsibilities

  • Schedule patient visits in the EHR to optimize the RN and APP's time while coordinating with patient and caregiver needs
  • Answer inbound calls, text messages, and voicemails in a professional and timely manner
  • Monitor and manage inbound and outbound faxes
  • Enter data, scan files, and print files to support patient care and maintain updated records in the EHR
  • Identify and manage health plan insurance for residents
  • Support the RN and Advanced Practice Provider by coordinating patient care to drive outcomes
  • Collaborate with local community resources that support patient care
  • Manage facilitated telehealth visits with Savoy's virtual provider via a tablet
  • Support ongoing care through direct resident visits
  • Manage communications and foster relationships with patients, caregivers, families, primary care providers, specialists, pharmacies, and senior living community staff
  • Drive engagement with eligible residents, POAs, and families through education to obtain signed consent forms
