About the Role
You will provide clinical and operations support to seniors in and around Lewiston / Moscow, ID & Pullman, WA. Working closely with an assigned Savoy Life RN and provider, you'll deliver healthcare to seniors across various settings including assisted living, independent living, memory care units, and individual homes. You'll handle back office support like scheduling patient visits, answering calls and messages, managing faxes, and maintaining EHR records. You'll also support care coordination by facilitating telehealth visits, conducting direct resident visits, and building strong relationships with patients, caregivers, families, and healthcare partners. This role involves travel between different senior living communities and homes throughout the day.
Requirements
- High School Diploma / GED (or higher)
- 2+ years of experience in healthcare with a focus on care coordination and healthcare administrative support
- Medical Assistant, Patient Care Technician, or equivalent certification preferred, otherwise 5+ years of healthcare experience
- Experience in geriatric care or senior living community is highly desirable
- Must possess a valid and current driver's license & be comfortable driving to different senior home settings
- Excellent electronic medical record (EMR) documentation skills
- Ability to juggle many tasks and switch between visiting residents and coordinating behind the scenes
- Flexibility to adapt to a dynamic and growing startup organization
Responsibilities
- Schedule patient visits in the EHR to optimize RN and APP time while coordinating with patient and caregiver needs
- Answer inbound calls, text messages, and voicemails
- Monitor and manage inbound and outbound faxes
- Enter data, scan files, and print files to support patient care and maintain updated records in the EHR
- Identify and manage health plan insurance for residents
- Support the RN and Advance Practice Provider by coordinating patient care to drive patient outcomes
- Collaborate with local Savoy team and other community resources to support patient care
- Manage facilitated telehealth visits with Savoy's virtual provider via a tablet
- Support ongoing care with direct RCC-led resident visits
- Manage communications and foster positive relationships with patients, caregivers, families, primary care providers, specialists, pharmacies, and senior living community staff
- Drive engagement with eligible residents, POAs, and families through education to get consent forms signed
