About the Role

You will act as a virtual health coach supporting pregnant patients and new mothers throughout pregnancy and postpartum. You will provide one-to-one and group education, work with the Care Delivery manager to deliver weekly engagement and coaching protocols, encourage participation, and help patients follow recommended care and tracking protocols.

Requirements

• DONA Birth, CD, or CBE; ICEA; CAPPA; or ProDoula certification.
• Illinois Medicaid Program Advanced Cloud Technology (IMPACT) certification.
• Communication and conversational skills.
• Experience with web and mobile technologies.
• Fluency in English.
• Experience in pregnancy, birth, and postpartum support.
• Unrestricted and independent authorization to work in the United States.

Responsibilities

• Execute weekly patient engagement and coaching protocols with the Care Delivery manager.
• Ensure engagement and participation among pregnant patients and new mothers.
• Help patients adhere to recommended care protocols and tracking requirements.
