Registered Nurse - Senior Living
Skills
About the Role
You'll provide hands-on clinical support for seniors across various settings including individual homes, assisted living, independent living, and memory care. You'll conduct in-person assessments and ongoing check-ins to identify care needs, and work alongside Advanced Practice Providers to develop and implement chronic condition care plans. You'll coordinate closely with primary care providers, specialists, POAs, families, community Health and Wellness Directors, and other care services to ensure seamless care delivery. You'll partner with an internal team that includes a local Resident Care Coordinator, a Partner Success leader, and a virtual Advanced Practice Provider. You'll keep detailed, accurate documentation in Savoy Life's EMR, triage after-hours patient phone calls, and act as an on-site clinical resource. You'll need to be comfortable operating with flexibility and adaptability as this role and the organization evolve.
