About the Role
You will provide hands-on clinical support for seniors across a variety of settings including individual homes, assisted living, independent living, and memory care communities, working up to 20 hours per week during business hours. You'll conduct assessments, ongoing check-ins, and in-person care visits to identify resident needs, and you'll assist Advanced Practice Providers in developing and implementing chronic condition care plans alongside the clinical team. You'll coordinate care by collaborating with primary care providers, specialists, POAs and families, community Health and Wellness Directors, and other care services to ensure seamless care delivery. You'll partner closely with a local Resident Care Coordinator, a Partner Success leader, and a virtual Advanced Practice Provider, and you'll keep accurate documentation in the Electronic Medical Record system. You'll also triage after-hours patient phone calls and serve as a clinical resource for on-site staff, all while staying flexible and adaptable in a growing startup environment.
Requirements
- Active Registered Nurse (RN) license in Washington
- Must possess a valid and current driver's license
- Strong background in geriatric/senior care, with a focus on care assessments and care coordination
- Experience in care planning for chronic conditions, including collaboration with interdisciplinary teams
- Excellent documentation and electronic medical record-keeping skills
- Ability to work autonomously, manage ambiguity, solve problems, and adapt to new processes
- Experience in working in senior living facilities is highly desirable
Responsibilities
- Conduct assessments, ongoing check-ins, and other in-person care for residents within senior living communities and other senior settings
- Assist Advanced Practice Providers in developing and implementing chronic condition care plans in collaboration with the clinical team
- Coordinate care for residents through collaboration with primary care providers, specialists, POAs/families, and other care services
- Partner with an internal team including a local Resident Care Coordinator, Partner Success leader, and virtual Advanced Practice Provider
- Maintain accurate and detailed documentation in the Electronic Medical Record (EMR) and other technologies
- Triage after-hours patient inbound phone calls to further support care
- Provide clinical expertise and act as a resource for on-site staff to support high-quality resident care
- Operate with flexibility and adaptability in a new and evolving role within a growing startup organization
